Bloated Goldfish not Dropsy

I have 2 indoor goldfish both 5 yrs old. One healthy. The other well ....

So approx. 18 months ago it started to bloat. Gradually. So fins don't stick out etc.
It feeds fine. And it actually swims about pretty good.
The bloating got to approx. size of a golfball if not a little bigger. So it literally looks like a golf ball with a head and fins. The size peaked about 10 mths ago.

Everything at that time pointed to a tumor but like said its been a total of 18 months now and its still fine.

I tried Epsom salt baths numerous times - did not decrease bloat.

I did have a topic on another site but the forums seem to have gone.

Please note as I am entering this from work so I did not have a picture to hand - however I have attached a picture I found on the Internet of an identical fish problem.

Any idea what else this could be ?

So tech info -
Its in a tank approx. 36 inches by 12 inches
Same tank for last 5 years.
Just 2 fish in the tank.
One fish healthy.
The other symptoms started about 18 months ago. But about 10 mths ago peaked and not got bigger or smaller.
Fins are NOT sticking out.
Swimming although a little difficult for it - is good.
Feeding - sinking pellets twice a day.
There is plant life in the tank.
Water change once a week 50% using the tetra tap conditioner as well
Full tank clean once a month
Has a pump filter and also a aeriation stone
